NURSING:
RNs and LPNs - responsible for the coordination, planning, implementing, and evaluating the nursing services provided to each resident.

TMAs - will administer medications to residents under the supervision of a licensed nurse.

C.NAs - provide assistance to residents in all aspects of daily living and strive to provide a home-like environment

MDS Coordinator - coordinate the completion of the comprehensive and Medicare required assessments with the rest of the ID team.

Medical Records Coordinator - maintain the facility medical records. Staffing Coordinator/Secretary - responsible for the management of daily nursing staffing needs and provide secretarial support for the nursing department.

 

 

SOCIAL SERVICES:
Social Worker - responsible for admission coordination, care planning/programming, resident and family intervention and discharge planning. Provides supportive services to assist residents’ adjustment to the emotional aspects of their illnesses, treatment and stay in the facility.

 

 

LIFE ENRICHMENT:
Assistants - responsible for planning and leading resident activity programs and resident care planning.

Drivers - provides safe transportation for residents to and from medically related appointments and activity events.

 

 

DIETARY:
Cooks and Aides - responsible for the preparation and service of meals for Pine Haven residents, Evergreen Place residents, Meals-on-Wheels recipients and special functions.

 

 

ENVIRONMENTAL SERVICES:
Housekeeping - responsible for the cleanliness of the building including resident rooms, restrooms, support areas, furnishings, linens and resident’s personal items/clothing.

 

 

MAINTENANCE:
Responsible for preventive maintenance, repair and operation of building, mechanical and electrical systems, equipment and grounds.

 

 

EVERGREEN PLACE:
Home Health Aides - responsible for providing personal care and delegated services designed to maintain the client’s physical and emotional well being.

 

 

ADMINISTRATION:
Administrator - manage all aspects of operations for Pine Haven so that the maximum quality of life will be provided to residents living in the Care Center and Evergreen Place. Finance Director – responsible for maintaining the financials of the facility. Receptionist - Customer Service position responsible for performing office/clerical duties. Human Resources - position responsible for performing personnel duties.
